Pregnancy Back Pain, Hip Pain and Pelvic Girdle Pain at Night: The Pillow Setup That Works (India 2026)

You did not sleep badly because of the mattress. You slept badly because for six hours your pelvis was twisted about fifteen degrees out of neutral, and nothing in the bed was holding it straight.

TL;DR: Pregnancy hip and lower-back pain at night is usually caused by the top leg dropping forward while side-lying, which rotates the pelvis and loads the sacroiliac joints for hours. The fix is three pillows, not one: (1) a firm knee pillow between the knees to keep hips stacked — The White Willow SecureComfort Knee Rest, ₹899; (2) a small roll or contoured pillow under the waist to fill the gap between ribs and hip so the lumbar spine does not sag; (3) a bump support in front so the abdomen is not pulling you forward — C-shape from ₹1,499. If pain is in the pubic bone or groin (pelvic girdle pain / SPD), the knee pillow is the priority purchase and keeping the knees together while turning matters more than any pillow. Persistent or severe pain needs a physiotherapist, not a pillow.

Why does my hip hurt so much at night during pregnancy?

Three things are happening simultaneously:

  • Relaxin softens the ligaments around the pelvis in preparation for delivery. Joints that were previously stable now allow more movement — and more strain.
  • Your centre of gravity has moved forward, increasing the lumbar curve and the load on the lower back even before you lie down.
  • Side-lying without support lets the upper leg fall across the body, internally rotating the hip and torquing the pelvis. Hold any joint at end-range for six hours and it will complain.

Only the third is fixable with bedding — but it is the one doing most of the damage.

What is the correct pillow setup for side-sleeping in pregnancy?

Think of it as building a splint around your torso.

Position What goes there Why The White Willow option
Between the knees Firm contoured knee pillow Keeps hips stacked and pelvis level; the single highest-impact change SecureComfort Knee Rest — ₹899
Under the waist Small roll or half-moon Fills the rib-to-hip gap so the lumbar spine does not sag sideways HalfMoon Harmony — ₹899
In front of the belly Bump wedge or C-shape arm Stops the abdomen dragging your torso forward into rotation Blissful Bump Embrace — ₹1,049
Behind the back Bolster or U-shape rear arm Prevents rolling flat onto the back overnight Blissful Bump Cradle (U) — ₹1,799
Under the head Correctly lofted side-sleeper pillow Shoulder-height loft keeps the cervical spine neutral DualRest Side & Back Support Contour Pillow — from ₹1,699
Under the calves (before bed) Leg elevation wedge Reduces evening swelling and heaviness LegRest Platform — from ₹2,099

You do not need all six. Start with the knee pillow. Add the waist roll if the lower back still aches. Add bump support past 24 weeks.

How firm should a knee pillow be for pregnancy?

Firmer than feels comfortable in the first thirty seconds. A soft pillow compresses to nothing under the weight of a leg within an hour, and you wake at 2 a.m. with the hip pain you tried to prevent. The correct test: press it hard with your palm and it should push back, not bottom out. High-resilience and high-density memory foam do this; polyester fill does not.

Height matters too. The pillow should be roughly thick enough that your top knee sits level with your hip, not above or below it. For most adults that is 6–8 inches of uncompressed thickness in the knee zone.

What if the pain is in the pubic bone or groin?

That pattern — sharp pain at the front of the pelvis, worse when parting the legs, worse turning in bed — is typically pelvic girdle pain, sometimes called SPD. The rules change slightly:

  1. Keep the knees together during every movement, including getting in and out of bed. Turn like a log, knees clamped.
  2. The knee pillow is essential, not optional — it maintains that separation-free alignment overnight.
  3. Use a smooth bedsheet so you can pivot without asymmetric effort.
  4. See a women's health physiotherapist. Pelvic girdle pain responds well to targeted treatment and support belts. A pillow manages the night; it does not treat the condition.

Does the mattress matter, or is it all pillows?

Mostly pillows — but an extremely soft mattress lets the heavier hip sink and re-creates the exact rotation you are trying to prevent. If your mattress is old and dipped, a firmer sleep surface will help more than another pillow. We covered this in why the surface matters more than the mattress.

What about daytime back pain from sitting?

Pregnancy back pain is rarely confined to the night. If you are working through your pregnancy, a lumbar backrest changes the posture you carry into bed. The TotalSupport Full Back Pillow from ₹1,199 or a Circle Ease Donut Pillow from ₹1,349 for tailbone pressure both help. See our back support guide for office chairs and WFH.

Frequently asked questions

Which pillow is best for pregnancy hip pain in India?

A firm contoured knee pillow such as the SecureComfort Knee Rest at ₹899. It addresses the mechanical cause — pelvic rotation from the top leg dropping — more directly than any full-body pillow.

Should I put a pillow under my bump or between my knees first?

Between the knees, in almost every case. Bump support improves comfort; knee support corrects alignment. If you can only buy one, buy the knee pillow.

Can a pregnancy pillow make sciatica worse?

An overly thick knee pillow that pushes the top hip higher than neutral can aggravate nerve symptoms. Aim for knee level with hip, not above it. If leg pain, numbness or weakness persists, speak to your doctor or physiotherapist.

Is it safe to sleep semi-upright to relieve back pain in pregnancy?

Many mums find a slight torso incline more comfortable in the third trimester, and it also helps reflux. Use a proper bed wedge rather than stacked pillows, and stay tilted toward your side rather than flat on your back. More in our wedge height guide.

How long before the pillow setup makes a difference?

Most people notice a difference within three to five nights. If nothing has changed after two weeks of consistent use, the problem is likely not positional and is worth raising with your obstetrician or a women's health physiotherapist.
